We do not ask for, and you should not submit, medical records or health information through this website.

This website is not a HIPAA covered service. It is not designed to receive medical records or protected health information. If an engagement proceeds, records are exchanged only by secure email under separate written instructions.

Medical records are accepted only by email to the HIPAA-covered address above or by the secure upload link I send; records are never accepted through the website forms, which are for contact details only.
